This Agreement applies to programs provided by Young, Black, and Suburban Kids (“YBS Kids”). “Participant” means the child listed on the membership form.

Participation and Responsibilities

• I authorize the Participant to attend YBS Kids workshops, mentoring, community-service projects, cultural and leadership programs, social events, field trips, and other announced activities.

• The Participant will act respectfully, follow safety instructions, and comply with YBS Kids policies. Bullying, harassment, fighting, threats, weapons, illegal drugs, alcohol, tobacco, vaping, and intentionally unsafe behavior are prohibited.

• I will provide accurate health and emergency information, disclose needed accommodations, follow arrival and authorized-pickup procedures, and promptly report changes. YBS Kids may restrict, suspend, or end participation when reasonably necessary to address behavioral, health, attendance, or safety concerns.
  
  Risks and Liability Release

• I understand that participation may involve risks, including injury, illness, allergic reactions, weather exposure, transportation accidents, actions of others, property loss, and delays in emergency assistance. I voluntarily accept the risks that are inherent in and reasonably related to participation.

• To the fullest extent permitted by Texas law, I release YBS Kids and its directors, officers, employees, authorized volunteers, agents, and representatives from claims arising from participation, including claims based on ordinary negligence. This release does not cover gross negligence, reckless or intentional misconduct, or rights that cannot legally be waived.
  
  Emergency Care

• If an emergency occurs, I authorize YBS Kids to provide basic first aid, contact emergency services, share relevant health information with responders, arrange transportation for treatment, and obtain reasonably necessary medical care if I or another emergency contact cannot be reached promptly.

• I accept responsibility for medical expenses not paid by insurance or another responsible party.
  
  Transportation and Field Trips

• Parents and guardians are responsible for transportation unless YBS Kids provides different written instructions. When transportation is arranged by YBS Kids, activity details and any additional permission requirements will be communicated in advance. Participants must use available seat belts and follow safety instructions.
  
  Media Authorization

• With separate permission on the membership form, YBS Kids may use approved photographs, video, audio, artwork, or statements featuring the Participant on its website, social media, newsletters, printed materials, reports, presentations, fundraising materials, news coverage, and approved partner communications.

• Media permission is optional and declining it will not affect participation. Unless separately authorized, YBS Kids will use only the Participant’s first name or a general description and will not intentionally publish sensitive personal information.

• Permission applies to the current membership term and may be withdrawn for future uses by emailing info@ybskids.org. Previously published or distributed materials may not be retrievable.

  This Agreement is governed by Texas law. If one provision is unenforceable, the remaining provisions will continue to the extent permitted by law. Electronic acceptance and signature have the same intended effect as paper acceptance and signature.
